Step 1. go to dafont.com and download the font "visitor"
Step 2. open a new photoshop document - 300x20?
Step 3. choose a gradient - preferably whitexcolor or lightercolorxdarkercolor
Step 4. Apply gradient from the bottom to top.
Step 5. Create a new document 6 px by 6 px
Step 6. Use brush tool and apply a straight line across
Step 7. Save the document as a pattern
Step 8. Return to your original document and create a new layer
Step 9. Apply the pattern and lower the opacity to 20%
Step 10. Create another layer using circle marquees <selection> only half way. Fill it with white. Lower opacity to 30%

Extra Step:
If you want the line effects... simply use pen tool and create a wave-like line. Then change your brush to 1-3 px and use "stroke" > "brush". After, simply put outter glow of a similar color to your lines.... duplicate... and transform. Rotate them around and make it look nice.
